We Are Currently Accepting New Patients Of All Ages

Prior to your appointment, you will be asked to complete patient information forms. Having these completed before you arrive will greatly reduce your processing time at your appointment.

If you are coming in for allergy testing, read this important information.

Your examination will begin with a thorough evaluation of your medical history. This is very important: please be prepared to discuss any past illnesses, past surgeries, prescribed or over-the-counter medications the patient is taking as well as the details of the current illness.

Please arrive at the clinic at least 30 minutes prior to your scheduled appointment time to complete any necessary paperwork.

No Smoking

For the health of our patients and employees, smoking is prohibited on clinic grounds.

No Food and Drink

Out of courtesy to our patients who may have food allergies, we request that you do not bring any food into the clinic.

No Perfumes and Colognes

Out of courtesy to those patients who are sensitive to odors or allergic to cosmetics, we ask that you do not wear perfume, colognes or scented lotions on the day of your visit to the clinic.
